Evaluation Form

The Evaluation Report is key for measuring our impact as a charity.

Please complete within 30 days of seeing the performance. The final 20% of the grant will be paid when the fully completed report is received. If your report is not received within 60 days of the performance we reserve the right to withdraw our grant for the outstanding amount.

Helpful resources

Collating feedback from a number of students, sometimes from a number of schools can be challenging for theatres and community groups. To help you with completing the Evaluation Form, we’ve created some resources to download for students and parents to fill in. Print, scan and upload the completed files to this form.

What to prepare for the Evaluation Form.

To help you prepare, here is a summary of the sections that appear in the Evaluation Form. We’ve outlined the requirements for each, so that you know what to expect. You can also save the form as you go and return to it later to complete each section.

  • In this section we will ask for basic contact information and an overview of the performance you saw, so we can connect it with your initial grant application.

  • In this part of the Evaluation Form, please provide us with feedback quotes from teachers or parents/guardians. We are particularly interested to hear reflections from teachers and parent/guardians on the experience as a whole and their perspective on the impact it has had on the children attending.

  • In this section, there is a space to provide feedback quotes from the children who attended the production. You can write this in the text box provided.

    Or alternatively, there is also space to upload any additional files (documents or vox pops) containing feedback quotes from children. We’re keen to hear about the educational and wider impact on the children and what they enjoyed rather than a review of the production.

  • In this section, we request event and production photography. Please provide photos of the children at the event pre or post show (not during the production) and any photos of their wider experience if relevant including any Q&As or workshops.

    If you’re a theatre applicant, we welcome production photography to be uploaded in addition to any photos collated from the schools.

    In the form, we ask you to confirm that you have obtained parental or guardian consent for the use of any photographs or videos featuring children, which may be included in our promotional materials. We will not publish any names.

    Please be aware that we may use these photos or supporting documentation within our future marketing.

  • There is a final optional section, where if relevant, applicants can upload files featuring written work or drawings that have been created within the classroom setting following the production, to demonstrate how the theatre trip has been featured in any follow up lessons or educational activities.

    Theatres may upload any educational resources for schools that they provided to support the production.
