Student Impact Evaluation Form

The Student Impact Evaluation Form* is key for measuring our impact as a charity.

All grant recipients are required to complete this form within 30 days of seeing the performance. The final 20% of the grant will be paid when the fully completed report is received and reviewed. If your report is not received within 60 days of the performance we will withdraw our grant for the outstanding amount.

Collating feedback from a number of students, sometimes from a number of schools can be challenging for theatres in particular. To help you with completing the Student Impact Evaluation Form, we’ve created some free optional resources to download for students and parents to fill in about their theatre trip.

Distribute/print, scan and upload the completed files to this form or use these as a template when requesting feedback from schools.

What to prepare for the Student Impact Evaluation Form.

To help you prepare, here is a summary of the sections that appear in the Student Impact Evaluation Form. We’ve outlined the requirements for each, so that you know what to expect.

You can also save the form as you go and return to it later to complete each section.

The form should be submitted by the initial grant applicant. Schools, Theatres and Touring Companies should collate feedback from students and teachers. Theatres/Touring Companies should submit the form once on behalf of the schools who attended the production as part of the grant with the collated feedback.
